Mission and Core Value

Our mission is to motivate and encourage people living with Diabetes, Endocrine and Metabolic disorders, as well as healthcare professionals, to work together in the planning, integration and delivery of high quality, effective and innovative patient centred care for the people of the Macarthur region and surrounding areas.

MDEMS is a multidisciplinary service with a predominantly ambulatory focus, although a wide range of inpatient and hospital-based activities are undertaken.

Diabetes

The diabetes educators and allied health staff are only able to support patients that are under the care of the MDEMS clinics, due to limited resources.

The diabetes service is supported by the highly successful Diabetes, Obesity and Metabolism Translational Research Unit (DOMTRU) led by Distinguished Professor David Simmons. Metabolic Services

The South Western Sydney Metabolic Rehabilitation and Bariatric Program (SWS MRBP) is a specialist multidisciplinary program for people with BMI ≥40kg/m2 and related health conditions and is based in Camden Hospital. There is a separate referral criteria and pathway for this program with significant wait times due to high demand.

The SWS MRBP is supported by a research program, the Macarthur Clinical Obesity Research Program (MCORP) which aims to identify the physical and mental health needs, mechanisms and treatment outcomes for excess weight and related comorbidities, in people with class 3 obesity (Body Mass Index of ≥40 kg/m2).

Paediatric Services provided

The paediatric team provides a multidisciplinary service for children and adolescents from 0-16 years of age with diabetes or endocrine conditions.

The team consists of Paediatric Endocrinologists, Diabetes Educators, Diabetes Dietitians, a Psychologist and a Social Worker.

Patients with Type 1 diabetes or presumed Type 1 diabetes should be directed to the Emergency Department at Campbelltown or Liverpool Hospital.

Patients requiring urgent review should be discussed with the Paediatric Endocrinologist on call through the Hospital Switchboard.

Patients with weight issues should be referred to the Growing Healthy Kids Program.

Other endocrine and diabetes referrals should be emailed or faxed.


Referral Process (for Health Professionals only)

Referrals for most clinics are now transitioning to e-referrals via Consultmed. Please click here to download a copy of the referral form.

All referrals are triaged based on criteria as listed on health pathways. For specific referral criteria, particularly type 2 diabetes and case conferencing, please see criteria below and follow Health Pathways.

Referrals for the Metabolic Program are to be sent via email or fax.

Referrals for gestational diabetes, pregnancy in pre-existing diabetes as well as endocrine disorders in pregnancy are sent to Maternity services via email or fax.

Referrals for the multidisciplinary High Risk Foot Service is via email or fax.

Type 2 Diabetes and Case Conferencing

A General Practitioner may refer ANY patient with Type 2 Diabetes. Exclusions for referrals to this service are patients who:                  

  • Are pregnant
  • Have an active foot ulcer
  • Paediatric 
  • Are seeking a driver’s license renewal
  • Are pre-diabetic
  • Have a Type 1 or gestational diabetes diagnosis
  • Clearance for surgery
  • On renal dialysis
  • Fitness to work forms
  • Are currently seeing a private endocrinologist                                                                           

If the patient meets the criteria, General Practitioners can complete the Integrated Diabetes Care Case Conference Referral Form, along with submitting the patient’s health summary and up to date pathology. Patients referred to case conferencing will generally receive a medical review within 4 weeks.

If the patient does not meet case conferencing criteria, please follow the Diabetes Specialist referral criteria above.
