Behavioural Assessment and Observation Form (BAOF)

The Behavioural Assessment and Observation Form (BAOF) was developed due to a gap analysis identifying that some basic needs for mental health consumers were not being met and therefore proactive measures were not being taken to prevent episodes of escalation and absconding.

The Paediatric BAOF, in particular, was designed due to the high number of paediatric patients presenting to hospital who require specialist mental health or behavioural care.

Here are several presentations that can be used as a guide of not only how to use the Paediatric BAOF, but also complementary education, provided by experts in their field, of why the form is required.
Some of the presentations are include but are not limited to:

  • Sensory processing
  • Child protection and development trauma
  • Medications
  • Self-harm and suicidal ideation
