COPD Chronic Care Program

Chronic Obstructive Pulmonary Disease (COPD) usually occurs in people who have smoked cigarettes. Exposure to irritants like dust or fumes can also increase the risk of developing COPD. COPD causes shortness of breath, cough, wheeze. Patients with COPD may experience frequent chest infections. Treatment is usually coordinated by the general practitioner (GP), with specialist assistance and hospital admission where required. 

Liverpool Hospital has a specialist respiratory liaison nurse to manage patients diagnosed with COPD at home.
 

Services provided:

  • Home visits and phone calls by a specialist respiratory nurse
  • Liaison between GP and other health care providers
  • Patient and family education
  • Assistance to access appropriate community services
  • Development of action plans to manage COPD exacerbations

Eligibility to the Program:

  • Must be 18 years or older
  • More than 2 emergency presentations or admissions per year
  • Resides within the Liverpool LGA
  • Able to engage in self-management strategies
  • Respiratory Physician, GP and patient agrees to participate
