Other services available in the community

Home and Community Care (HACC) services
The HACC program includes a wide range of services that aim to support people who need some help to continue living at home. The services may be offered by big organisations – like Meals on Wheels or Home Care – or smaller ones like neighbourhood centres or multicultural community organisations. The type of support that is provided by HACC services includes:
  • housework
  • personal care like showering and dressing
  • delivered meals
  • in-home and centre-based respite
  • social support programs
  • transport to medical appointments

Level 1/2 package (previously known as Community Aged Care Packages)
The purpose of these packages is to provide low level care to clients with complex needs in their own home. It is aimed at people who would otherwise be suitable for hostel level care. Level 1/2 packages are designed to assist older people to live at home with independence, confidence and support. Each package is individually planned and coordinated. A person needs to have been assessed by the Aged Care Assessment Team (ACAT) as being eligible for the program.
 
 
Level 3/4 package (previously known as Extended Aged Care in the Home)
These packages are similar to the Level 1/2 above but for people with higher needs, who would otherwise need high level residential care. Each package is individually planned and coordinated. A person needs to have been assessed by the Aged Care Assessment Team (ACAT) as being eligible for the program.
