Hospital-linked services provided in the community

Aged Care Assessment Team (ACAT)
A multidisciplinary team that provides comprehensive assessment, information and advice to frail, older people who want to remain at home with support or who are considering living in a residential aged care facility. Assessment by the ACAT is a formal requirement for entry into residential aged care facilities (both permanently and for respite) and community support programs such as Community Aged Care Packages, Extended Aged Care in the Home program and the Transitional Aged Care Program. A person may be referred to an ACAT from a wide range of sources including their family or carer, a service provider, their general practitioner or other health professionals or they may refer themselves. Specialist Aged Care and Rehabilitation Team
This service provides multi-disciplinary assessment and goal-directed treatments for people living at home. It caters to people aged 18 years and over who have a physical disability and people aged over 65 years who have complex physical and/or cognitive disability or falls. A centre-based service is available at Liverpool Hospital. A home based service is available across Liverpool & Fairfield Local Government Areas for those unable to attend a centre-based service due to severe mobility, cognitive or behavioural restrictions.
 
Respite services
Respite can be offered either through centre-based programs or within people’s homes. Centre-based programs provide a structured small-group environment for frail older people or older people with dementia. There are dementia specific, frail aged specific and Aboriginal groups available. Programs run by the Local Health District closest to Liverpool are at Hoxton Park, Fairfield and Carramar. There are a number of centre-based as well as in-home programs offered by organisations other than the Local Health District and information about these services can be found here
 
Transitional Aged Care Program (TACP)
TACP provides a package of services to patients who have been in hospital and who – at discharge – require a combination of “low intensity” therapy and home support services. The TACP may be provided either in a person’s home or in a residential aged care facility. The aim of the program is to improve a patient’s functioning and avoid the possibility of needing to return to hospital. The program is time limited (maximum of 12 weeks) and based on helping each person work toward their individual goals. A person needs to be assessed by the Aged Care Assessment Team (ACAT) while they are in hospital to be eligible for the program.
 
 
Dementia Advisory Service
Provides advice, education, counselling, and support to clients with dementia and/or their carers. The service facilitates networking and education to service providers concerned with the care of people with dementia and their carers. Home visits can be conducted to provide information, education and support, and community education sessions organised as required.
 
Community Dementia Clinical Nurse Consultant (CNC)
The role of the Dementia CNC is to improve outcomes for people with dementia, their families and carers through clinical advice, consultation, education and support of other health professionals who are providing dementia care.
 
Specialist Mental Health Service for Older Persons
Provides a range of services including assessment, treatment and management for older people affected by mental health disorders and/or severe behavioural disturbance.  The services also assist families and carers of people experiencing these problems.
