How to access the services

Accessing services run by the Local Health District

Out-patient Geriatrician Clinic
A referral needs to be written by a General Practitioner and then contact made with the Clinic Administration Officer on 8738 4868.
  
Specialist Mental Health Service for Older Persons
To get information or make a referral to this service, please call the Mental Health Line on 1800 011 511.

HACC services
The Home and Community Care (HACC) program offers a range of services to assist people to continue functioning in their home and community. The type of services includes: housework, showering and personal care, community transport, help with shopping and Meals on Wheels. For general information about HACC services go here.  Information about local services that might be appropriate for you can be found at the Support Services Directory on the website of the South West Sydney Ageing and Disability Support Services.
 
Aged Care Australia
This front end service is provided to improve access and navigation of the aged care system. This service provides information about eligibility, services provided and costs for a range of aged care support programs. You can call 1800 200 422 or go to the Aged Care Australia website.
 
Commonwealth Respite and Carelink Centres
Commonwealth Respite and Carelink Centres provide a point of contact for the general public, carers, service providers, general practitioners and other health professionals to access information on carer support and respite services. Phone 1800 200 422 during business hours, or for emergency respite support outside standard business hours, call 1800 059 059.
 
After Hours GP Helpline
If you or someone you care for has an urgent health concern at night or over the weekend and you are not sure what to do, there is an after hours GP helpline that can give you the reassurance and practical medical advice you need. Phone number: 1800 022 222
Operating times are: 6pm to 8am Monday to Friday; 6pm Friday to 8am Saturday; 12noon Saturday to 8am Monday; and Public holidays. More information about the service can be found here
