The information given in these surveys provides important information about your health that may influence your care. You can decide not to fill in the survey and you will still receive the best care possible.
Before your Clinic appointment, and then every 30 days. The 5-10 minute survey is initially on an iPad in clinic, and then you can choose to complete it at home.
You need to have an appointment with a healthcare team member, and your MRN (Medical Record Number). The first survey will be completed on an iPad in the clinic area, but after that, you can get them sent to your home email address or to your phone.
There are 9 common symptoms that occur for cancer patients: (Pain, tiredness, nausea, depression, anxiety, drowsiness, appetite, well-being and shortness of breath). We ask you to rate each from 0 (no symptoms) to 10 (the worst possible severity).
This scale measures distress from 0 (no distress) to 10 (extremely distressed). The checklist identifies what is causing your distress, practical, family, emotional, spiritual and physical problems.
Distress is an unpleasant feeling and may interfere with the ability to cope with cancer, the physical symptoms and treatment. Distress ranges from common feelings of vulnerability, sadness and fears to problems that can be disabling, such as depression, anxiety, panic, social isolation and existential and spiritual crisis.
Asking you how you feel, gives your health care team a focus for conversation about the best care for you.
Your survey becomes part of the medical record that is available to your health care team in real time.
The answers you give in the survey may result in you receiving further information about the things you are concerned about.
The surveys will soon be in multiple languages, until then, professional interpreters are available if you need help to communicate with staff. The service is free and confidential. Our staff can book the interpreter for you, or you can call the Translating and Interpreting Service on 131 450 if you need to speak to us before your appointment.
