Advance care planning and end of life care

The ACRU has a strong interest in advance care planning and end-of-life care. The Unit published a detailed report that explored current practice and opportunities to improve practice in advance care planning and other aspects of end-of-life care. The project involved interviews with managers of aged care facilities in South Western Sydney. The project provided important links into service development work within the Area Health Service.

The ACRU has also been responsible for developing a comprehensive area-wide program to promote advance care planning called My Wishes. This program includes policies, medical record forms, a wide range of information sheets and an informative website. The Unit has recently been funded to develop a nationally-focused website on advance care planning in dementia.
