About

The Department of Dietetics provides nutrition assessment, education and support for inpatients and outpatients at Liverpool Hospital. The department is made up of Clinical Dietitians and Dietitian Assistants. Clinical Dietitians are members of the Dietitians Association of Australia http://daa.asn.au

Dietetic Outpatient Services include:

  • Diabetes
  • Swallowing and Nutrition
  • Cancer Therapy
  • Renal
  • Gastrointestinal
  • Paediatrics
  • Home Enteral Nutrition

Referrals for overweight, obesity and Type 2 Diabetes are recommended to contact the South Western Sydney Medicare Local for a comprehensive health program. Contact 1300 179 765 or http://www.swsml.com.au 

Referrals
If you wish to see a Dietitian and are an inpatient at Liverpool Hospital you can ask your Nurse to contact the Ward Dietitian.

For outpatient bookings, please contact the Department of Dietetics directly. You must have a current referral from your GP and live within the Liverpool Local Government Area to be eligible for Outpatient services.

Digital Referral Pathways to SWSLHD

South Western Sydney Local Health District receives outpatient referrals through Consultmed, our secure online referral platform. This helps referrals reach the right service and supports timely review and processing.


Referrals can be submitted in one of three ways

1. For your GP or other Healthcare Provider

Please speak with your GP, specialist, allied health professional or primary care provider and ask them to submit your referral through Consultmed.

Healthcare providers can create a free, secure account or log in using the link below.

2. For NSW Health Staff

NSW Health staff can submit referrals to SWSLHD services through Consultmed.

Staff can log in securely using their StaffLink credentials.

3. For Patients or Carers

If you already have a referral letter, you can upload it directly through the SWSLHD self-referral portal.

Important Information

Please make sure your referral includes:

  • Your personal details
  • The reason for referral
  • Relevant clinical information
  • Supporting documents such as imaging or test results

Once submitted, your referral will be reviewed by the relevant hospital team. You may be contacted if additional information is required.
