If you receive a referral letter from your doctor for a consultation at a Fairfield Hospital outpatient clinic, you will need to contact the clinic directly to make an appointment. We will try to provide you with an appointment as soon as possible and at a time convenient to you.
Some clinics may request that you forward a copy of your referral prior to scheduling your appointment. Staff will inform you of this when you initially contact the clinic.
Please make staff aware if you are using Community Transport to and from your outpatient appointment
You are welcome to ask any questions or discuss concerns you may have during your appointment. Please let us know if you have changed your address or phone number, so we can contact you if necessary.
Once you have arranged your appointment, some clinics may confirm it with you by phone and/or confirmation letter.
Depending on the Outpatient Clinic, on the day, you may experience a delay as some people with special needs may be seen first. Please inform staff if you are experiencing a long wait or would like to leave the clinic until it's time for your appointment.
Arriving at your Appointment
On the day of your appointment, please allow time for travel, parking and locating the clinic. Arriving late for your appointment may result in your appointment being rescheduled.
Changing or cancelling your appointment
If you are unable to keep your appointment or would like to change your appointment time or do not require any further appointments, please advise staff as soon as possible so we can offer the appointment to someone else.
If you fail to attend an appointment, you and your referring doctor will be notified and you may be discharged from the clinic.

On the day of your appointment, please bring the following items:
If the appointment is for your newborn child, you must register your child with Medicare before attending your appointment at the hospital. If you have not registered your child and do not have a valid Medicare number, fees may apply.
Please note that some clinics may have additional requirements that will be provided to you when you make your appointment or attend the clinic.
Please let us know prior to your appointment if you require an interpreter and we will endeavour to arrange this service for you.
Wheelchairs are available from Fairfield Hospital at the Main Entrance and Emergency Department. These cannot be pre-booked, so please allow at least 30 minutes waiting time for this service.
