Simone Proft

Director, Strategy and Partnerships

 

This role provides leadership in defining the strategic course for the District ensuring a sharp focus on long term strategic priorities and that shorter-term plans are aligned with these directions with overall responsibility for the day to day management of the Strategy and Partnerships Directorate (SAPD).

The Director manages a team of staff in supporting a large number of innovations, major high cost contracts, tenders and model of care redesign. This position also manages a strategic portfolio, the Affiliated Health Organisations and the ministerial funded non-government organisations, inter-district clinical programs and a large number of strategic partnership projects.

Kim Jobburn

Program Manager, Strategic Programs

This role provides strategic and operational leadership of the Palliative Support Package (PEACH), the SWSLHD NGO/AHO Programs and the Greater Western Sydney Health Partnership, working in close partnership with external service providers, LHD executive staff and through both local specialist and primary care providers in each LHD, and key stakeholders in delivering these programs.

This role is responsible for providing high level project & program administrative support to meet the objectives of the Strategy and Partnerships Directorate (SAPD)

Manager NGO/AHO Programs

This role provides leadership and management of the contracts, agreements, performance and partnerships of the 19 NGOs in the NSW Health NGO Grant Program, including the development and maintenance of relationships between NGOs and relevant program areas in SWSLHD.

Consumer and Community Participation Unit (CCPU) Manager

This role manages the SWSLHD Consumer and Community Participation Unit (CCPU) and is responsible for developing, implementing, marketing, monitoring, evaluating and advocating for consumer and community participation (CCP) across SWSLHD

Executive Assistant, CCPU

This role provides a full range of executive and administrative support and co-ordination for the Consumer and Community Participation (CCP) Unit.

Project Manager, Strategic Projects

This role provides leadership and manages the roll-out and implementation of projects across SWSLHD.

Project Manager, Strategic Projects

This role provides leadership, advice and manages the implementation of PRMs (HOPE IT), LBVC initiatives, OXIDIST, HITH Comms Pilot, h-trak and other strategic projects across SWSLHD.

Project Officer

This role leads, manages and supports the implementation of  SAPD projects in SWSLHD.

Joanna Rae

Manager, Planning Unit

This role is responsible for leading service and strategic planning for the District. The role works in close partnership with internal and external stakeholder relationships to develop well-aligned and contemporary planning documents for facilities and services across the District.
