Community Palliative Care Service (CPCS)

Palliative Care puts the person and their family at the centre of care. It is for people living with an advanced illness (a condition that cannot be cured and will worsen over time), where the main goal is to make life as comfortable as possible.
Palliative Care can include:

  • Taking care of pain and symptoms
  • Supporting people with cancer (oncology patients)
  • Planning for future medical decisions
  • Providing counselling and help with grief (bereavement)

Who can use this service?

The Community Palliative Care Service supports people of all ages, including:

  • Newborns, babies and children
  • Teenagers and young people
  • Adults and older adults

We support people living with life-limiting illness such as:

  • Congenital disorders (conditions a person is born with)
  • Neurodegenerative or neuromuscular diseases (conditions that affect the brain, nerves or muscles and get worse over time) - we work together with the neurodegenerative specialist team for these
  • Frailty, multiple health conditions and/or dementia
  • Advanced cancer
  • End-stage chronic disease (a serious long-term illness in its final stages) with complex symptoms

What can you expect?

We visit people in their home and give care from the time someone joins our service through to end of life. Our team looks at all of a person’s needs, including:

  • Pain
  • Other physical symptoms
  • Emotional, social and spiritual health
  • Support for family and carers

People living in Residential Aged Care Homes (RACH) can also receive this care. Our nursing team will visit and work with the GP and RACH staff to create a care plan.

How to make a referral

We accept referrals from:

  • GPs and specialists
  • Hospitals (public and private, including those outside of South Western Sydney Local Health District)
  • Residential Aged Care Homes
  • Other services such as non-government organisations
  • Clients and/or their families and carers - please note: we recommend you discuss this service with your doctor before you contact us

To make a referral, please contact the Triple I Hub. They are open from 7:00am - 10:00pm, every day (7 days per week). Once a referral is processed, the Triple I Hub sends it to the right Community Palliative Care nursing team.
