Palliative Care puts the person and their family at the centre of care. It is for people living with an advanced illness (a condition that cannot be cured and will worsen over time), where the main goal is to make life as comfortable as possible.

What can you expect?
We visit people in their home and give care from the time someone joins our service through to end of life. Our team looks at all of a person’s needs, including:
People living in Residential Aged Care Homes (RACH) can also receive this care. Our nursing team will visit and work with the GP and RACH staff to create a care plan.
How to make a referral

To make a referral, please contact the Triple I Hub. They are open from 7:00am - 10:00pm, every day (7 days per week). Once a referral is processed, the Triple I Hub sends it to the right Community Palliative Care nursing team.
