About Triple I
Triple I is a point of contact for patients, carers and health professionals who need referrals to Primary and Community Health services. We intake and refer into the following services:
- Aboriginal Chronic Care Program (ACCP)
- Acute and Extended Care in Community (Formally known as Community Nursing)
- Care Finder
- Community Older Persons Intervention and Liaison Outreach Team (COPILOT)
- Community Wound Assessment Team (COMWAT)
- Healthy Kids and Families (Formally known as Child and Family Health)
- Hospital in The Home (HiTH)
- Liverpool/Fairfield Specialist Aged Care and Rehabilitation Team (SpACRT)
- Out of Hospital Care (OHC) - End of Life (EOL)
- Out of Hospital Care (OHC) - Safe and Supported At Home - (SASH)
- Out of Hospital Care (OHC) - Community Packages (ComPacks)
- PEACH Program (Palliative Care Home Support Packages)
- POST FOBT Direct Access Colonoscopy Clinic
