About Triple I

Triple I is a point of contact for patients, carers and health professionals who need referrals to Primary and Community Health services. We intake and refer into the following services:

  • Aboriginal Chronic Care Program (ACCP)
  • Acute and Extended Care in Community (Formally known as Community Nursing)
  • Care Finder
  • Community Older Persons Intervention and Liaison Outreach Team (COPILOT)
  • Community Wound Assessment Team (COMWAT)
  • Healthy Kids and Families (Formally known as Child and Family Health)
  • Hospital in The Home (HiTH)
  • Liverpool/Fairfield Specialist Aged Care and Rehabilitation Team (SpACRT)
  • Out of Hospital Care (OHC) - End of Life (EOL)
  • Out of Hospital Care (OHC) - Safe and Supported At Home - (SASH)
  • Out of Hospital Care (OHC) - Community Packages (ComPacks)
  • PEACH Program (Palliative Care Home Support Packages)
  • POST FOBT Direct Access Colonoscopy Clinic
