About Our Directorate

Ageing Well in Community supports people aged 65 years and over, Aboriginal and Torres Strait Islander people aged 50 years and over, and adults aged 18 years and over living with a neurodegenerative disease. Our aim is to help people maintain their independence and quality of life while living in the community.

We provide a range of government funded assessment, restorative care and support services.
Our Goal is to make sure older persons in south west Sydney have access to assessment and restorative care services in the community.

Our Mission is to support older people living in south west Sydney by:

  • Assessing their needs and helping them access available resources and services
  • Working in partnership to tailor care and support to each person’s needs & goals

We provide care primarily in the community, in people’s homes.

Our Services

Aged Care Assessment – south west Sydney

Provides comprehensive assessments for older people living in the community who may require access to government funded aged care supports, including:

  • Support at Home packages
  • Approval to access permanent or respite placements within Residential Aged Care facilities (RACF)
  • Short term therapy services such as Transitional Aged Care Program (TACP)

Short Term Therapy Services

We provide allied health and nursing services funded through programs such as:

  • Transitional Aged Care Program (TACP)
  • Commonwealth Home Support Program (CHSP)

These services are designed to time-limited supports which focus on client’s achieving their goals, optimise their independence and improve wellbeing.

Neurodegenerative Disease Support

Specialised support for adults aged 18 years and over living with neurodegenerative conditions, excluding Parkinson’s disease or dementia, living in the community.

All services operate Monday – Friday 8:00am – 4.30pm, excluding public holidays.

Aged Care and Assessment Services
Aged care assessments to determine eligibility for accessing Government funded aged care supports 

  • Commonwealth funded home care packages 
  • Services such as a meal assistance, community transport, personal care, or social support
  • Short-term restorative care to reverse or slow difficulties with everyday tasks and support older people living at home
  • Funding requirements for residents in Residential Aged Care Facilities

Allied Health Reablement Service
Short-term Allied Health services for older people living at home

  • Dietetics (nutrition and diet)
  • Occupational Therapy
  • Physiotherapy
  • Speech Pathology
  • Social Work
Referrals can be completed via the My Aged Care website, or by calling
My Aged Care on 1800 200 422 .
NB: Ability to accept referral can be dependent on capacity.

Transitional Aged Care Program
Short-term support for older people returning home after a hospital stay 

  • Goal setting
  • Allied Health support
  • Nursing support
  • Support with personal care and domestic tasks
Internal Allied Health staff to complete referral form and email to:

NB: Referrals can only be accepted from inpatient staff

Neurodegenerative Disease Supportive Care
Community support for people with progressive disease that damages the brain or nervous system  

  • Allied Health services
  • Patient and carer education
  • Nursing support
  • Respiratory management
  • Symptom management
  • Specialist support and advice
