Aboriginal Chronic Care Program

The Aboriginal Chronic Care Program (ACCP) supports Aboriginal and Torres Strait Islander people at-risk of developing a chronic condition.

ACCP will help you to:

  • Understand your chronic condition
  • Look after your chronic condition
  • Keep track of your appointments
  • Set and work towards achieving health goals
  • Get referrals to other health services as needed e.g. Mental Health, Nutrition, Drug and Alcohol, Social Work, Women's Health
  • Help with finding other government services as needed e.g. Centrelink, Housing

Anyone can make a referral to the Aboriginal Chronic Care Program.

What is a chronic condition?

A chronic condition is an illness that lasts for longer than 6-months.
Some examples of a chronic conditions are:

  • Arthritis (joint pain and stiffness)
  • Cancer
  • Cardiovascular disease (issues with the heart and blood vessels)
  • Diabetes (high blood sugar levels)
  • Heart disease (problems with the heart)
  • Kidney disease (problems with kidneys)
  • Osteoporosis (weak bones)
  • Respiratory disease (lung issues)
