Outcomes that matter to patients

The Patient Reported Measures program aims to enable patients to provide direct, timely feedback about their health related outcomes and experiences. This will drive improvement and integration of healthcare across NSW.

The Agency for Clinical Innovation (ACI) PRMs Program will enable direct and timely feedback to healthcare providers about patient reported health outcomes and experiences of care to help drive improvement in care delivery, quality and safety, and to facilitate the integration of health care across NSW.

 > What are Patient Reported Measures (PRMs)?


Patient reported measures (PRMs) are surveys which capture outcomes and experiences that matter most to patients, helping us to understand what matters most to patients and to find out if the care we deliver supports the outcomes and experiences that patients expect.

Patient Reported Measures includes two types of surveys:

1. Patient Reported Outcome Measures (PROMs) 2. Patient Reported Experience Measures (PREMs)

PROMs capture information about a patient’s quality of life or condition-specific measures.

Responses are directly reported, without interpretation by a clinician or anyone else.

PROMs can be quality of life or condition specific. PROMs are used to provide real time feedback to clinicians who use this information in care planning and decision making to provide timely person centred care.

PREMs ask patients to describe, rather than simply evaluate, what happened during their encounters with health services.

PREMs are completed anonymously and are used by sites and services to drive service change and quality improvement.

> Why use PRMs?


With today’s technological advancements, there is a lot we can tell about a patient without even talking to them (e.g. their heart rate and blood pressure, and recent admission history). However, unless patients (and/or their carers) are asked about what is important to them and how they rate their quality of life and experiences of healthcare, we don’t know the whole picture.

The evidence demonstrates that clinical indicators often fail to correspond with how a patient is feeling. Therefore, it is important to routinely ask for the patient's perceptions of their own health and wellbeing, at the point of care.

This can be achieved through the systematic collection and use of patient-reported measures (PRMs). There is good evidence to demonstrate that patients who are more engaged in their healthcare tend to choose less costly interventions (e.g. presenting to a physiotherapist for lower back pain instead of hospital emergency).

PRMs have also been well documented to support clinician decision-making and shared care planning. They are a good indicator for overall patient outcomes; especially in those conditions marked by a person’s level of health and wellbeing, rather than their risk of dying.

The NSW Agency for Clinical Innovation (ACI) PRMs Program aims to enable patients to provide direct, timely feedback about their health related outcomes and experiences. This will drive improvement and integration of healthcare across NSW.

Patient Reported Measures have been collected across NSW for a long time in a range of forms. As we move towards value based healthcare, to guide and support a more consistent approach in the state-wide implementation and collection of PRMs NSW Health have developed a Patient Reported Measures Framework (www.health.nsw.gov.au/Value/Documents/prm-framework.pdf).

The framework will help align existing initiatives and platforms, so we can use PRMs across the system to give patients a greater say in their care.

> Information Technology | HOPE Platform


Patient reported measures (PRMs) can be captured electronically using personal computers, tablet devices or smartphones, enabling patients (or their families/carers) to self-report on experiences and outcomes.

A purpose-built IT solution has been co-designed with consumers, clinicians and managers across NSW in partnership with the ACI, eHealth NSW and the NSW Ministry of Health. The system, known as Health Outcomes and Patient Experience (or HOPE) supports the routine collection, measurement and timely reporting to clinicians.

 

Health Outcomes and Patient Experience

To date, Patient Reported Measures have been collected in a variety of ways, including on paper or on iPads via the legacy IT Platform.

In 2020, a purpose built IT solution was co-designed with consumers, clinicians and managers across NSW in partnership with the ACI, eHealth NSW and the NSW Ministry of Health.

The system known as Health Outcomes and Patient Experience (HOPE) will support the routine collection, measurement and timely reporting of PRMs to clinicians.

HOPE has been designed to meet the needs of patients, healthcare providers, managers and services.

HOPE will enable patients to access and complete surveys electronically at home using personal computers, tablet devices or smartphones. If they prefer, patients will also be given the option to complete the surveys on an iPad when they attend the service or clinic.

HOPE is being implemented in a phased approach across NSW from early 2021.
