Time is Precious Program (TIPP)

Advanced Care Planning and End of Life Care in the Community

The Time is Precious Program supports people who are living with life-limiting illness. We help patients stay at home and feel safe in the community.

We work with patients, families and carers to:

  • talk about what matters most
  • make informed choices about future care
  • plan ahead for end-of-life (EoL) needs

Our team includes:

  • a nurse practitioner
  • a clinical nurse specialist
  • an occupational therapist
  • a social worker

How we can help

You can be referred to our service for:

  • help to manage symptoms (when specialist palliative care is not needed)
  • checks for how you manage daily tasks (also known as functional assessments)
  • support with Advanced Care Planning
  • help to connect with local services
  • emotional, social or spiritual support
  • support for families and carers

Who can use this service 

You can use this service if you:

  • are 18 years or older
  • live in south west Sydney
  • are likely to be in the last 12 months of life (as advised by your doctor/health care team)

This service may not be right for you if you:

  • already have specialist palliative care support
  • need urgent medical treatment
  • need high-level medical or nursing care that cannot be safely provided at home
  • need inpatient care for complex emotional or social needs

How to refer

*Please note, we currently only accept internal referrals from SWSLHD P&CH services.

Referrals are made through the Triple I Hub. Please complete the referral form and email to:  SWSLHD-TripleI@health.nsw.gov.au
