Hospital in the Home

The Hospital in the Home (HITH) program helps patients leave the hospital early or avoid going altogether. With HITH you get to choose to be treated at home or in a clinic by nurses from the community.
HITH includes:

  • Getting medicine through a needle in your arm (Intravenous therapy)
  • Taking antibiotic or other medications
  • Treating of conditions such as deep vein thrombosis or pulmonary embolism

If the hospital or your doctor thinks the HITH program can work for you, they can make a referral.

Who can join:

To join the HITH program, you must:

  • Live in South Western Sydney (refer to map)
  • Have your doctor or the hospital say you are well enough to be treated at home or in a clinic
  • Have a safe home with enough space for treatment
  • Have a phone 
  • Have support from a carer or family support when needed 
  • Be able to care for things like a cannula or catheter 

What happens next:

  • If you have a script for medication, go to your local pharmacy right away
  • A nursed from Community Health will call you to do ask some questions
  • If you can go to a clinic, you will get an appointment
  • If you are not able to go to a clinic (e.g. you might have trouble moving or not have any transport), a nurse will visit you at home. Community Health Nurses work from 7:30am – 8:00pm
  • A nurse can teach you, your carer or your family, how to take your medication

The number of home or clinic visits you get will depend on what treatment you need. 

The nurse will let you know if there are any changes to your appointments. If you are not able to make an appointment, please contact Triple I on 1800 455 511.

To make a referral, please contact Triple I. They are open from 7:00am - 10:00pm, 7 days a week.

If your condition does not improve, you should contact local hospital ED for advice.
