The Community Older Persons Intervention and Liaison Outreach Team (COPILOT) helps older people in the community with non-life-threatening conditions to avoid hospital visits.
COPILOT assists older people who:
- Live in a residential aged care facility (RACF)
- Live in an independent living unit
- Live in their own home
COPILOT can help with:
- Pneumonia (lung infection)
- Bowel and bladder issues (problems using the bathroom)
- Cognitive decline (memory problems, confusion or trouble thinking clearly)
- Delirium (sudden confusion)
- End of life care and planning
- Preventing falls
- Broken bones
- Malnutrition (poor diet)
- Taking care of pain
- Taking lots of medicines (Polypharmacy e.g. using five or more medicines at the same time)
Referrals to COPILOT can only be made by a health professional, NSW Ambulance, or a hospital. HealthDirect and residential aged care facilities can also make referrals to COPILOT.
Who is eligible:
To access COPILOT, you must:
- Live in South Western Sydney Local Health District (refer to map)
- Be over 65 years of age or 50 years of age for Aboriginal and Torres Strait Islander patients
- Be medically safe to be managed at home or in a RACF
- Require short term urgent assessment or intervention to prevent a hospital visit within the next 48 hours
- Be a high risk of admission or readmission to a hospital
Who is not eligible:
- Patients that can access other community services or are being seen by another service that meets the need for referral
- Patients who need a rehabilitation program or Transitional Aged Care Package (TACP)
- Patients with a health condition that needs urgent help e.g. sepsis, stroke

COPILOT is available from 8:00am - 4:30pm, 7 days a week. Phone consultations are available from 4:30pm – 8:00pm.
