Perinatal Family Conferencing (PFC)

Perinatal Family Conferencing (PFC) is a model of care for pregnant woman and their families where there is risk of significant harm concerns for the unborn baby. The model of care is collaboration between South Western Sydney Local Health District (SWSLHD) and South Western Sydney District Communities and Justice (DCJ).


The model of care supports early engagement with women in pregnancy to address risks and reduce the number of infants taken into statutory out of home care (or identification of an appropriate carer prior to birth) and promotes a philosophy of empowerment and participation for parents and family in decision making regarding their child.


In the Perinatal Family Conferencing we aim to have three meetings for the pregnant woman where we focus on strengths and engaging the family in problem solving and ensure a case plan is developed to address identified issues, recorded and signed off by all parties on the day.

All referrals to the Perinatal Family Conferencing program must come directly from Department of Communities and Justice (DCJ).


The pregnancy Family Meetings are facilitated to support you and to identify strengths, concerns and any additional support you may need during your pregnancy or once your baby is born. During the meeting this is done by the use of the “Three Houses” to help guide the discussion.

 

Three key questions are asked and everyone is asked to contribute:

  • House of worries
    • What are we worried about in relation to the baby and the family?
  • House of good things
    • What is working well in the family?
  • House of dreams
    • What needs to happen to make sure the baby is safe in the future?
