Patient Reported Data collection at Liverpool Rheumatology Department

The Rheumatology department collects information about your clinical condition on forms that you are asked to complete before each visit to your doctor. This information is used to assist your doctor to understand how your illness is affecting you during your consultation.

If you consent for this information also to be used for research, it will be de-identified and entered into the department research database.

There will be no impact on the care you receive from any of the staff in the department if you do not consent to the use of your data for this purpose, but the forms will still be used to assist with your clinical care.
