The Clinical Information Department is dedicated to providing you with easy access to your medical information in accordance with NSW Health Policy and the Health Records and Information Privacy Act [HRIP].

Following the application process we can provide you with either a:

Copy of your medical record [clinical notes]
Dates of Attendance letter
Discharge Summary
Workcover Certificate
Confirmation of Birth letter
Medical Certificate
Medical Report
