INTEGRATION AND PARTNERSHIPS

Services

The Integration and Partnerships Directorate (I &P Directorate) was conceived in response to the need for greater integration and coordination of care within Primary & Community Health and associated services and in response to the potential for greater involvement of external providers anticipated in the future, as the health and social care environment shifts to models for contestability of services. The objectives of the I & P Directorate are to:

  • Support an integrated care environment across the community, acute, and primary care settings (including external providers)

  • Support the effective and sustainable engagement of external providers to maximise benefits to patients and to SWSLHD

  • Ensure ongoing improvement and accountability of delivered and contracted services, as well as considering that the delivery of services is undertaken by the most appropriate provider of care

The directorate adds value across Primary & Community Health by facilitating shared planning and commissioning, coordinating engagement with external providers and supporting performance monitoring functions of the Primary & Community Health enterprise as well as relationship management for partners in service delivery.
