Specialist Intellectual Disability Health Team (SIDHT)

 

The Specialist Intellectual Disability Health Team (SIDHT) has 6 teams across NSW to improve the experience of care for people with intellectual disabilities.

About SIDHT:

In South Western Sydney Local Health District, SIDHT does three main things:

  • We create plans to manage unresolved health problems for people with intellectual disability
  • We help other healthcare providers by offering advice and support to better manage their patients' health needs
  • We give expert advice to clinicians and general practice (GPs) on how to assess and care for people with intellectual disabilities – for example, helping build skills, confidence, and experience

Most of SIDHT's work involves supporting healthcare providers to improve services and achieve better health outcomes for people with intellectual disabilities. We do this by making reasonable adjustments to a patients' healthcare plan and improving pathways to access care.

Who can access SIDHT:

The SIDHT team in SWSLHD offers a time limited service for people with intellectual disabilities who:

  • Have complex health needs, AND
  • Have a current health problem not met by usual care, AND
  • Have a referral from their current health professional.
