Child and Family Medical Services

Community Paediatrics provides community based non-acute services for children, adolescents and their families. Availability of the following services varies across SWSLHD.

Child Developmental Assessment Service

Referrals accepted from:

• Private and public paediatricians
• Paediatric neurologists
• Allied health (from within SWSLHD) (eg speech pathologist, occupational therapist, psychologist)

Refer by sending completed referral form to:

• Faxed to (02) 8738 4800
• Emailed to SWSLHD-ComPaedDevAssess@health.nsw.gov.au

Community Paediatric Clinics for Vulnerable Children and Youth

Referrals accepted from:

• GPs
• Paediatric medical staff
• Allied health

Refer by sending completed referral form to:

• Emailed to SWSLHD-ComPaedIntake@health.nsw.gov.au
