Community Health Nursing

All Hospital Referrals (acute Phase) post hospital admission.
Acute/Post-Acute referrals (HITH- Hospital in the Home)/ from hospitals and GP’s to community initiation of intravenous antibiotic therapy.
Community Wound assessment team (ComWat) assisting in clinical assessment and consultation in Residential Aged Care Facility (RACF) or General Practices (GPs) with regards to chronic and transitioned wound care clients.
Non-acute nursing chronic referrals from hospitals, GP’s via My Aged Care (MAC).
Palliative Care, on call support.
Palliative Care Home Support Packages (Peach) Program.
Trial of Void (TOV) / Catheter care in community and RACF.


Aboriginal Chronic Care Program (ACCP) Nursing and Allied Health

A program to assist aboriginal and Torres Strait Islander people to obtain primary health care as required, provide care coordination services to eligible Aboriginal and Torres Strait Islander people with chronic diseases who require coordinated, multidisciplinary care and improve access for Aboriginal and Torres Strait Islander people to culturally appropriate mainstream primary care.


Integrated care for people at risk of hospitalization (ICPROH)

A program to assist people at risk of frequent hospitalisation to obtain primary health care as required, provide care co-ordination services to people with chronic diseases who require coordinated, multidisciplinary care and improve access to culturally appropriate mainstream primary care.


ComPacks

Low level, non-clinical and case management program of community care for people being transferred home from a participating New South Wales Public Hospital. Each package is available for up to 6 weeks from the time of their transfer home.


Safe and Supported at Home (SASH) Program

A time limited, community based and goal oriented support intended to enable clients to remain safely at home, reducing avoidable admissions to hospital and residential care.

    Eligibility to the program:
  • Aged 18-64
  • Applying for or appealing an NDIS decision or have been declined by NDIS
  • Medicare eligible

Bankstown: Speech Pathology and Neuropsychology
Liverpool/Fairfield: Social Work, Speech Pathology, Neuropsychology, Occupational Therapy and Physiotherapy
Macarthur: Upper limb
Bowral: Occupational Therapy, Physiotherapy, Social Work, Speech Therapy, Continence.


Child and Family Referrals

Universal Health Home Visits (UHHV) early childhood clinic appointments, group sessions such as sleep and settling, breast feeding, parenting groups.
