Paediatric to Adult (P2A) Transition Care Service

 

About P2A:

The Paediatric to Adult (P2A) program assists people aged 14-24 years transition from child to adult health services.​
We look at all the issues that might impact a young person so that we can give the best care, advice and support. For example, we can help with:

  • Finding and getting referrals to adult health services,
  • Education to help build skills, knowledge and confidence, and
  • Working with the young person, their paediatric (child) and adult healthcare teams to make sure there is a smooth transition, and all their health needs are met.
