Out of Hospital Care Programs

 

The Out of Hospital Care (OHC) Program assists clients to safely discharge from hospital. It aims to prevent clients from returning to hospital by offering short and medium-term case management and packages of non-clinical care.
For example, non-clinical care can include:

  • Help with tasks around the home (domestic assistance)
  • Help with light meal prep
  • Transport to medical appointments
  • Support for carers (respite care)
  • Social support

Our Patients:

Our clients can include:

  • People who have recently discharged from a public hospital in NSW and live in south west Sydney (refer to map)
  • People who live at home in south west Sydney and need immediate case management and non-clinical support to prevent going into hospital

These packages offer low to medium levels of care which include non-clinical case management and home care services. Eligible patients may also transition between package types depending on their changing needs.
