OUT OF HOME CARE (OOHC) PROGRAM

The purpose of the Out of Home Care Program is to improve the health outcomes for children and young people in OOHC. Children and young people referred to the OOHC Program receive a primary health screen which covers; growth, development and psychosocial issues. From this primary health screen children and young people are referred for further interventions as required.

This service is part of a state-wide network of services funded by the NSW Ministry of Health and is primarily an outreach or home visiting.  All referrals to the OOHC Program must come directly from Department of Family and Community Services (FaCS).
