How to get a record of your COVID-19 vaccination

Proof of your COVID-19 vaccination will be recorded on your immunisation history statement after you’ve had your vaccine. Once you have received all required doses, you will also be able to access your COVID-19 digital certificate. If you are unable to access your COVID-19 Immunisation Certificate and you were vaccinated at one of the following vaccination clinics, please complete the below form:

  • Bankstown PCYC Vaccination Centre
  • Catherine Park Community Centre
  • Liverpool Vaccination Centre
  • Prairiewood Vaccination Centre
  • South Western Sydney Mobile Vaccination Clinics
  • South Western Sydney Vaccination Centre - Macquarie Fields

Be sure to enter your details carefully so we can match them against our COVID-19 Vaccination records and finalise your certificate.

If you were vaccinated somewhere else, please contact the provider that administered the vaccine or call the Australian Immunisation Register on 1800 653 809 between 8am and 5pm, Monday to Friday.
