High Risk Foot Service

Access to Services

To be eligible for an appointment, patients must reside in the areas serviced by South Western Sydney Local Health district. They must be over 18 years of age and meet our eligibility criteria:

  • Foot ulceration (acute or chronic)
  • Suspected foot infection (including foot cellulitis and osteomyelitis)
  • Suspected or diagnosed acute Charcot foot

Access to the High Risk Foot Service requires a referral from a general practitioner or medical officer. We do not accept walk appointments. Once a referral has been made, please contact to book an appointment. 

Referrals should include:

  • The reason for referral including duration of the concern, previous treatment, current medication and mechanism of injury
  • Medical history
  • Any relevant investigations and information, such as pathology results (blood tests and swabs), imaging (x-rays, other imaging), vascular studies
  • Up to date contact information for the patient
  • Other relevant information including if an interpreter is required and if they are ambulant

If you have an urgent concern, please ring the service. Urgent appointments can be provided within 24-48 hours.

Inpatient referrals can be submitted through eMR.

There is no cost to attend the HRFS.
